Patient Forms

Before your first visit, there is some preliminary information that we will need to collect from you. These forms can either be provided to you in person when you arrive for your first visit or can be completed ahead of time by by filling them or downloading from the links below. These forms can either be emailed to us or brought with you to your first visit at our office.

Preparing For Your First Appointment

In order to make the best decisions about your dental care, please bring as much information about your dental and medical history as possible including:


  • Dental Insurance card and benefit booklet with yearly limits or exclusions
  • Medical Insurance card
  • A list of all medications and what conditions were they were prescribed for
  • The name and phone number of your physician and specialists
  • The name and phone number of your previous dentist
  • Visa, Mastercard, Debit, Cash or Cheque and debit are accepted as forms of payment for the portion not covered by insurance


If you are coming to us from another dental office, we will be happy to contact them and have them send us your most recent X-rays and dental history. Please click the New Patient link and fill out the records transfer form.

What to Expect at Your First Appointment

Your first appointment will take about one hour. Because each patient has unique needs and concerns about their oral health, we will start by getting to know more about you and your oral health goals. During your first appointment, we will perform a thorough initial examination that will allow us to develop an individualized treatment plan to address your specific concerns and help you achieve the best oral health possible.

During your examination, we will:

  • Perform a head and neck exam
  • Screen for oral cancer
  • Take your blood pressure
  • Check that your jaw joints (TMJ’s) and muscles are working in harmony with your teeth
  • Evaluate your teeth, checking for decay, fractures, wear, looseness, and other issues
  • Assess your periodontal (gum) tissue health
  • Scan your teeth with a radiation-free digital 3D scanner
  • Take digital radiographs and use Carivu to assess for decay and other potential problems (unless you have current radiographs from a previous office)
  • Take digital photographs to use as a diagnostic aid, to help address aesthetic concerns and to help you understand what is happening within your mouth
  • Perform additional diagnostic and screening tests as necessary

Treatment Recommendations

After the initial exam has been completed, we will discuss the findings of the examination with you using

3-dimensional images, digital radiographs and photographs. We encourage you to ask any questions that come

to mind so we can help you make an informed decision about your dental care. Then, in collaboration with

you, we will create a personalized treatment plan with options that are appropriate for you.

Once your treatment plan has been finalized, we will discuss the timing and cost of your treatment with you and set up any future appointment(s) for your dental care.  Usually, we start with an appointment with a dental hygienist for your initial hygiene care.

If your treatment recommendations are complex, we may need more time to review your examination results.  In this case, we would bring you back for a complimentary consultation appointment.

Treatment Timing

We understand that it may be necessary for some patients to extend their treatment over a period of time. In these cases, we can provide a schedule that is comfortable for you. 


Sometimes, due to financial constraints, we need to choose less expensive treatment options. However, these choices often involve some compromise in function, aesthetics, predictability, or longevity when compared to the recommended ideal treatment plan.
